Ordering Home Oxygen

There are two types of Home Oxygen Order Forms (HOOF) explained in detail below

Part A HOOF:

The Part A HOOF is for non-specialist healthcare professionals, or for temporary supply pending a specialist review. The Part A HOOF can only be used to order a static concentrator or static cylinders for use within the home.

Part B HOOF:

The Part B HOOF is a restricted document for Home Oxygen Assessment and Review Services (HOS-AR), Paediatric and other Specialist Teams.  The Part B HOOF can be used to order ambulatory oxygen equipment in addition to any Long Term Oxygen Therapy (LTOT) Equipment.

Before submitting a HOOF to Air Liquide Healthcare you should:

  • Carry out a risk assessment to ensure that the patient and, or carer understands the safety advice around the use of home oxygen, this includes the dangers of smoking cigarettes and e-cigarettes near to their oxygen equipment
  • Obtain patient consent to share their details with the oxygen supplier and other parties, including Fire and Rescue Services and electricity distributors.  To do this the patient must sign the Home Oxygen Consent Form (HOCF). You should store the HOCF within the patient’s record
